The game was inspired by the 2011 film Contagion and the 2008 Adobe Flash game Pandemic 2.[1] The player creates and evolves a pathogen to annihilate the human population with a deadly pandemic. The game uses an epidemic model with a complex and realistic set of variables to simulate the spread and severity of the plague. Plague Inc. is a strategy-simulation game in which the player indirectly controls a plague, which has infected patient zero. The player can choose between game modes and pathogens and complete the objective set by the game mode by evolving the plague and adapting to various environments. The player can use many different types of pathogens, each with its own advantages and disadvantages which influence evolution decisions. Initially, the player may only select bacteria. As the player completes various playthroughs in Plague Inc: Evolved, they'll unlock a bunch of genetic modifiers that essentially serve as upgrades to help improve a plague's infection rate, strengthen its mutations, and so on. Given the vast number of gene modifiers present in the game, it can take quite some time to unlock the whopping 25 Genetic Code variants present in the game.

Either way you play it, this simple looking, but freaking addictive game has some replay value. As the player wins at games and destroys more of the earth, each new game may reward the player by unlocking other new types of virus, or spores and various other nasty bugs and DNA strands to make an ultimate killing plague.
That replay value comes when the player decides to try a different strategies, such as keeping the virus from the far more horrific things and only working as a common cold perhaps, until such time as they release the mutations in the plague that start killing people. What about starting in a different country how will this effect things? Maybe try the game at different difficulty levels to really have to change the players approach at how the game is played.
The main game itself can be challenging, and on the higher difficulty levels it starts to hurt. People wash their hands properly, the sick are quarantined, governments require perennial blood tests of all citizens, and so on. More diseases and genes are unlocked with each successful genocide. There is also a scenario mode not unlike something you might see in a 'Civilization' game, where specific diseases or circumstances have been set up for players to solve.
